Amendment of name under section 27.
Age below 07 years
1. Declaration under Section 27. (The application for this can be made by the mother / father or guardian of the child concerned. The declaration form must be signed by the mother / father or guardian. It must be certified by the Justice of the Peace.)
2. Demand letter
3. Attested copy of birth certificate of concerned (obtained from Regional Secretariat)
4. An Affidavit (Declarant to be signed on Rs.50/- stamp)
5. A photocopy of the parents' marriage certificate.
6. A photocopy of the National Identity Card of the mother / father or the person giving the statement should be submitted.

Above 21 years of age
1. Declaration under Section 27 (must be certified by Justice of the Peace)
2. Demand letter
3. Attested copy of birth certificate of concerned (obtained from Regional Secretariat)
4. An Affidavit (Declarant to be signed on Rs.50/- stamp)
5. Village Officer Recommendation Letter (To be obtained by the Village Officer of the residential division.
6. Submit at least 03 of the following written evidence that the name has been used.
Newspaper Advertisements (Must be one year old.) Compulsory
Copy of bank account book (must mention the name to be amended)
Birth certificate of children (must mention the name to be amended)
Marriage certificate.(The name to be amended should be mentioned)
Education Certificates (name to be amended should be mentioned)
Sports Certificates (name to be amended should be mentioned)
Electoral Roll (name to be amended should be mentioned)
Copy of Deeds (name to be amended should be mentioned)
Appointment letters (name to be amended should be mentioned)
Student Work View (name to be revised should be mentioned)

1. The written evidence referred to in No. 06 must be one year old.
Amendment of name under section 27/a, 52/1.
01. Request letter regarding amendment.
02.Original copy of birth certificate of the subject.
03. Original copy of birth certificate of mother and father.
04. Original copy of marriage certificate of mother and father.
05. Photocopies of ID cards of parents (submit with ID cards)
06. Village Officer Residence Certificate (Regional Secretary to be certified)
07. Affidavit (Attested by Justice of the Peace)
08.Hospital birth record (only if date and place of birth are amended)
should be considered.
If there is a difference between the names in the marriage certificate and the birth certificate, separate affidavits should be submitted for the same. (According to the following format)
*Both mother and father must be present to give oral evidence on the date of submission of file in case of amendment to enter marital status.

03. Timings to be submitted :- 05 working days in the week. 9:00 a.m. to 1:00 p.m. till 3.00
04. Fees to be paid to avail the service :- Rs.50/= Govt Fees for Declaration
Time taken to provide service (Normal Services and Priority Services)
The letter file should be prepared and completed and sent to the district secretary's office. After approval and return, revisions should be made.

Photocopies of National Identity Cards of parents
Letter confirming residence of village officers (signed by P. L. Anu)
